1. Overview

Human cortical organoids provide an unprecedented opportunity to model early brain development; however, their growth and maturation remain constrained by diffusion-limited oxygenation and the absence of critical extra-neural interfaces that shape corticogenesis in vivo. During embryonic development, cortical expansion is regulated by coordinated interactions with invading vasculature and meningeal boundary tissues, which provide trophic signals, extracellular matrix components, and mechanical context. Although recent vascularized organoid systems improve metabolic support, they lack controlled perfusion dynamics and do not incorporate a physiologically relevant cranial boundary.

This project aims to engineer a perfused vascularized brain–meningeal assembloid-on-chip by integrating unguided human brain organoids with hiPSC-derived brain microvascular endothelial cells (hiBMECs) and meningeal cells within a compartmentalized microfluidic platform.

We hypothesize that combining controlled endothelial perfusion with meningeal boundary integration will reduce hypoxia, improve structural organization, and enhance neurodevelopmental fidelity compared to current vascular-only models.

2. Introduction

The human cerebral cortex develops within a tightly regulated microenvironment shaped by vascular invasion, cerebrospinal interfaces, and meningeal signaling. The meninges secrete trophic factors and extracellular matrix components that influence radial glial organization and cortical lamination. Concurrently, the developing vasculature establishes metabolic homeostasis, modulates oxygen gradients, and contributes to blood–brain barrier maturation. These coordinated interactions define the structural and functional architecture of the cortex. Despite remarkable progress in organoid technology, current models lack a perfusable vascular network capable of sustaining physiological oxygen delivery under shear stress. Additionally, the absence of a meningeal boundary removes an essential source of developmental signaling and mechanical constraint. While recent vascularized assembloids demonstrate improved metabolic profiles, they do not provide precise control over perfusion dynamics or compartment-specific media conditions. As a result, organoid heterogeneity and necrotic core formation remain persistent limitations.

This project addresses the following research questions: 1. Does controlled endothelial perfusion reduce hypoxia and improve viability in guided brain organoids? 2. Does incorporation of a meningeal boundary layer enhance structural organization and cortical marker expression? 3. Does combining perfusion with meningeal integration produce synergistic improvements in maturation and reproducibility?

We hypothesize that perfusion-mediated vascularization will significantly reduce hypoxic stress and necrosis relative to static controls; that meningeal integration will improve radial glial alignment and cortical layer specification; and that the combined system will yield superior synaptic maturation and structural fidelity compared to either intervention alone. Irrespective of outcome, this study will clarify whether cranial boundary biology and controlled transport represent critical determinants of organoid maturation. Positive findings would establish a next-generation human cranial neurovascular platform for developmental modeling and translational drug testing.

3. Methods

Guided brain organoids will be generated from genomically stable human induced pluripotent stem cell lines (KOLF2.1J and H9) using established self-patterning protocols. In parallel, isogenic hiBMECs will be differentiated using a recently established protocol¹. Pre-vascularized organoids will be generated through VEGF supplementation and co-encapsulation of embryoid bodies with hiBMECs, following a recently published method². hiBMECs and hiPSC-derived pericytes will be seeded into a defined vascular channel within a commercially available duplex microfluidic device that physically separates endothelial and neural compartments while allowing controlled exchange through a porous interface. Following endothelial stabilization, continuous perfusion will be initiated under physiologically relevant shear stress to promote lumen formation and vascular maturation. Vascularized brain–meningeal assembloids will be generated by adding primary human meningeal cells, which will to form a boundary layer surrounding the organoid within the neural compartment at defined developmental stages following a previous report. Four experimental conditions will be evaluated: static organoids, vascularized organoids without perfusion, meningeal co-culture alone, and fully integrated perfused cortical–meningeal assembloids³. A minimum of six independent organoids per condition per replicate will ensure adequate statistical power to detect biologically meaningful differences in viability and maturation metrics. Organoid development will be assessed through confocal imaging and quantitative immunofluorescence for endothelial (CD31, ZO-1), cortical layer (TBR1, CTIP2, SATB2), synaptic (HOMER1, PSD95) and meningeal (markers. Hypoxia will be quantified via HIF1α expression and necrotic core measurements, while metabolic viability will be evaluated using ATP and LDH assays. Endpoint multi-OMICs including proteomics and metabolomics (Monash University core facilities), will characterise neurovascular maturation and stress-response pathways on the same organoids.

Data will undergo standardized preprocessing, segmentation, and normalization prior to statistical analysis. One-way ANOVA with appropriate post hoc testing (or non-parametric equivalents when necessary) will compare conditions, with mixed-effects models accounting for batch variability. If perfusion reduces hypoxia and necrotic core formation, this will support the hypothesis that transport limitation is a primary constraint in organoid systems. Finally, we will compare our data with existing similar datasets. If meningeal integration improves cortical organization and layer specification, it will validate the importance of boundary signalling. Improved performance of the fully integrated system would indicate synergistic enhancement of maturation through combined vascular and boundary effects.

Experimental design

De-risking our methodological approach

To de-risk the proposed “PERFECT” platform, we will implement a staged and modular validation strategy that isolates and benchmarks each engineering component prior to full system integration. First, individual modules: organoid generation, endothelial perfusion, and meningeal co-culture will be independently optimized using well-characterized readouts (viability, hypoxia, barrier integrity, and lineage fidelity) to ensure robustness and reproducibility across iPSC lines. Second, the microfluidic perfusion system will be calibrated using defined flow rates and shear stress regimes validated against established blood–brain barrier models to prevent endothelial instability or shear-induced damage. Third, biological variability will be mitigated using isogenic lines, standardized differentiation timelines, and inclusion of multiple independent replicates with mixed-effects modelling to account for batch effects. Fourth, orthogonal validation (imaging, biochemical assays, and multi-omics) will ensure that observed phenotypes are consistent across modalities rather than assay-specific artifacts. Finally, predefined decision points (go/no-go criteria) will guide progression from single-module validation to fully integrated assembloids, ensuring that technical failure in one component does not compromise the entire platform. Collectively, this approach minimizes technical risk while maximizing interpretability, scalability, and translational reliability of the system.

The team

The proposed project will be undertaken at the Human Stem Cell Laboratory within the Department of Psychiatry, Monash University, led by Dr Agustin Cota-Coronado. This laboratory has established a novel human ‘brain-on-a-chip’ platform to model inflammation as a key environmental risk factor in psychiatric and neurodevelopmental disorders, integrating advanced iPSC-derived neural systems with microfluidic technologies.

The project is further strengthened by collaboration with Dr Henrique Nogueira Pinto (UMC Amsterdam, Netherlands), an emerging leader in bioengineering who has recently secured >€1 million in competitive funding to develop next-generation blood–brain barrier (BBB)-on-a-chip systems, bringing critical expertise in vascular modelling and microphysiological systems design.

In addition, Dr Kevin Law (Charles Perkins Centre, University of Sydney) contributes extensive expertise in the generation and functional characterization of diverse iPSC-derived neuronal and non-neuronal cell types, including brain organoids and multi-electrode array (MEA) analyses.

Together, this consortium represents a highly complementary team of early-career investigators with demonstrated capacity to deliver innovative, high-impact research. By combining strengths in stem cell biology, bioengineering, and functional neurophysiology, the team is uniquely positioned to advance organ-on-a-chip technologies and develop next-generation human models of neurodevelopmental and psychiatric disease.

4. Pilot Data (Optional)

We have successfully incorporated 3D spheroids in the NETRI microfluidic device that allows to have controlled innervation of different spheroid-like structures they are interacting. Additionally, this ‘brain-on-a-chip’ platform allowed to incorporate astrocytes and microglia in both spheroids. Finally, we were able to model inflammation which is well studied as an environmental risk factor to psychiatric disorders and neurodevelopmental disorders.

All required infrastructure, laboratory equipment, and cell culture resources are fully established and operational within the Human Stem Cell Laboratory at Monash University. This includes dedicated facilities for iPSC maintenance, neuronal differentiation, and microfluidic organ-on-a-chip experimentation, ensuring immediate project readiness and technical continuity.

In addition, Monash University provides access to state-of-the-art core facilities supporting multi-omics profiling, including transcriptomics, epigenomics, and proteomics, available at subsidized institutional rates. These integrated platforms ensure cost-effective, high-throughput data generation and enable comprehensive molecular characterization aligned with the project’s objectives.
